How to Use the Template

Follow these simple steps after downloading the PDF template to make your affidavit legally binding in the Philippines:

  1. Print Copies: Print at least three (3) copies on Legal (8.5" x 14") or A4 size paper. You will need one copy for the notary public, one for your records, and one for the entity (e.g., insurance company) where you will submit it.
  2. Fill in the Blanks: Using a pen with permanent ink (black or blue), fill in all the required information cleanly and accurately. Pay special attention to the detailed description of the jewelry and where it was lost.
  3. Do Not Sign Yet: Wait until you are in the physical presence of a Notary Public.
  4. Get it Notarized: Bring the printed and filled copies along with a valid Government-issued ID (Passport, Driver's License, PRC, etc.) to a notary public. Sign the documents in front of them, and ensure they stamp and seal it. Note: Notarization in the Philippines usually incurs a fee ranging from โ‚ฑ100 to โ‚ฑ500 depending on the location.
